Now that the general public accepts that LASIK vision correction

The US Food and Drug Administration has cleared to use Abbott's iFS Advanced Femtosecond Laser to create bow-shaped or curved arcuate incisions in corneal surgery, including cataract surgery.
The iFS Laser is currently used to create LASIK flaps and other corneal incisions in the eye and is the fifth generation of IntraLase technology, which has been used in more than 5 million procedures worldwide.
Nearly 3 million patients undergo cataract surgery each year in the United States. With this new clearance, the iFS Laser now gives surgeons the ability to make precise, bladeless arcuate incisions during surgery and customize the incision for each individual patient.
Factors such as placement, length, depth and radius of curvature can influence the surgeon's desired change to the cornea. These parameters can be difficult to control through traditional incisions made by hand using surgical knives. Now, using the iFS Laser, arcuate incisions can be performed with greater precision, predictability and customization than is possible with a manual technique.

Older eyes on Lasik
Tired of wearing glasses or contacts to help them see clearly, more and more people are turning to laser vision correction for a more permanent fix. For patients middle aged and older in particular, the surgery is becoming increasingly popular.
“It’s definitely been a trend,” said Dr. Vincent Marino, D.O. at LasikPlus in Kenwood. “We’ve been doing laser vision correction at LasikPlus since 1995; early on, we were typically seeing younger people as a general trend. As time has passed, we are definitely seeing more of a trend of people in their 40s and 50s, and even 60s.”
Contributing to the increase in popularity among that demographic is an increase in acceptance of the surgery among the general population, according to Dr. Gary Varley, medical director at Cincinnati Eye Institute in Blue Ash.
“Lasik is now a well known, fairly well understood procedure,” he said. “Ten years ago, patients would still say ‘That’s too new’ or ‘I don’t know anything about it.’ ”
And as they get older, patients are more likely to be able to afford the procedure, the average cost of which is about $2,100 per eye, he said. Plus, vision correction can become more and more of a hassle as we age.
“When they get over 40, now they need reading glasses,” Dr. Varley said. “If they have contacts, they have to get reading glasses; if they have glasses, they have to get bifocals.”
Unfortunately, laser surgery won’t mean leaving those reading glasses behind: Although it can correct farsightedness, nearsightedness and astigmatism, Lasik can’t fix presbyopia, the age-related loss of focusing that necessitates them.
Instead, Dr. Marino said patients can get the surgery to correct their eyes for distance and continue to use reading glasses, or they can try monovision correction, which is also becoming more and more popular.
“In one of the eyes, we correct them for distance; in the opposite eye, we leave them a little bit nearsighted, so they can use that eye for reading,” Dr. Marino said. “For individuals wearing monovision contacts, we can mimic that quite well.”
Having the surgery at an older age doesn’t pose additional risks, Dr. Marino said.
“Someone over 40, it’s just as possible for them to get a good outcome as it is for a younger person,” he said. About 75 percent of patients make good surgery candidates, he said.
What factors make someone a good or bad candidate? Dr. Varley lists the following:
• Degree of focusing error: The bigger the abnormality or the thicker the glasses, in general, the harder it is to get your vision as perfect as you want.
• General eye health (must be free of glaucoma, macular degeneration, cataracts and severe dry eye), shape of the cornea and size of pupils.
• The personality of the patient: “If you have a type A personality that (believes) everything is black and white, they may not be as happy with the outcome as someone who’s more easygoing and accepting,” he said.
General eye health can limit on older patients who want surgery, as they’re more likely to develop those diseases, Dr. Varley said.
For all patients, Dr. Varley recommends that they choose a surgeon and a center that makes them feel comfortable, and make sure their surgeon understands their goals.
“You can’t take LASIK back and say that it doesn’t work,” he added.

Incorporating iris registration increases the accuracy of delivery. Here the laser identifies landmarks on the iris, the colored part of the eye, and moves with the eye.
The modern day flap-making devices are more advanced than the freehand blades of previous centuries. Today we have automated thin flap makers and the laser to create consistent uniform flaps.
There have also been different advances in FDA laser machines, such as Visx S4IR and Allegreto, which are the top two.
So today’s laser vision correction surgery is safer, longer lasting and provides better vision, especially at night, without any glare or halos.

Simultaneous LASIK, corneal inlay implantation improve near, distance vision
ORLANDO, Fla. — Simultaneous treatment of presbyopia and refractive error with LASIK and corneal inlay implantation improved vision, particularly in older patients, according to a large study presented here.
"At 6 months, close to 90% of patients reported being satisfied or highly satisfied with their outcomes," Minoru Tomita, MD, PhD, said at the American Academy of Ophthalmology meeting. "After 6 months, about 95% of patients reported decreased dependence on their glasses."
The study included 2,000 patients who underwent simultaneous LASIK and implantation of the Kamra intracorneal inlay (AcuFocus) between 2009 and 2011. The inlay was implanted in only the non-dominant eye.
Patients were classified into age group by decade: 40s, 50s and 60s.
Study results at 6 months postop showed that mean uncorrected near visual acuity at 30 cm was J2 in the 40s and 50s age group and J3 in the 60s age group.
Patients in their 40s gained three lines of vision, those in their 50s gained four lines, and those in their 60s gained five lines, Dr. Tomita said.
Mean uncorrected distance visual acuity was 20/20 or better in all age groups, he said.
Concurrent centration of the LASIK flap, ablation zone and inlay implantation site was not particularly problematic, Dr. Tomita said in response to a question from an audience member.

Eye-tech solutions: New surgery could end the need for reading glasses
- Procedure costs £4,600 for both eyes
Millions of Britons who suffer from long-sightedness could now have their vision fully restored following groundbreaking treatment.
With an estimated 23 million experiencing partial difficulty with their ability to see, new procedures could now end the need for reading glasses.
Four million patients are now expected to have the operation over the next decade with the practice set to be made available to private patients as of tomorrow, which costs £2,800 for one eye and £4,600 for the pair.
The technique, developed by scientists, sees a microscopic ring placed in the cornea, which in turn allows only focused light to reach the retina, and subsequently improves vision for all distances.
More than 6,000 treatments have been carried out since last year in Europe and certain parts of Japan.
Dr David Allamby, an ophthalmologist is delighted with the progress in the attempt to cure impaired vision.
He said: Finding a treatment for presbyopia is important.

'This makes it harder for muscles to squeeze it into shape to get a clear image.
'Most over-45s will know that feeling: suddenly struggling to read menus or maps, but our longer sight for driving, say, remains good.
'By the age of 50, 90 per cent of your lens flexibility is lost.
'The only people who still read easily are those who were born short-sighted, but who already use glasses for distance.
'This is like granting immunity from ageing, at least for your eyes.'
Scientists have been monitoring a number of patients who six years ago were amongst the first to undergo the operation.
And results from the implants have shown no signs of any problems.
However warning signs have been raised for those over 70 who might be keen on the procedure.
Furthermore, chairman of the Royal College of Ophthalmologists, Larry Benjamin was sceptical, insisting:
'This is an interesting concept. But it won't suit everyone and I imagine certain professionals such as pilots, where night vision is important would not be allowed it.'
In the UK, Londoner Theresa Ferguson, 53, became the first Brit to undergo the procedure last month.
And she has spoken of her delight with the routine surgery saying: 'I had brilliant eyesight until I was 45. Then I began struggling. I need to read small print for work.
The health worker then described the immediate improvement in her vision.
'I was nervous, but it was painless, although uncomfortable. It took 15 minutes.'
The technique, developed in California cost $70 million in order to perfect, before being recently made available to the public worldwide.

New LASIK “Ask the Surgeon” Online Resource Launched
The choice to get LASIK surgery should be made only after educating oneself on the process, consulting with doctors, and getting an evaluation. As of the beginning of May 2011, questions can now be answered by experienced LASIK surgeons through the website seewithlasik.com.
Users can post questions through the “Ask LASIK Surgeons” button, which will be forwarded to the LASIK Editorial Advisors for review and comment. Answers will then be posted on a blog so that all users can benefit.
